Tracking symptoms can help your doctor — and you — better understand medical problems. I have a purse-sized notebook and a binder that I use for health information, but things started getting messy. I created my own symptom tracking sheet, which I’ve linked below, to help me stay organized.

Some symptom trackers are confusing and have too many options. I found that having a few blank boxes without lines or checkmarks works best for me. If you’re looking for a simple symptom tracking sheet, you may find this method works well for you too.

I’ll explain how I use this basic symptom tracking sheet, but feel free to do what works best for you. Let’s break down each section:

1) Symptoms

This is where I jot down a few words that describe my daily symptoms, such as:

  • Dizziness
  • Lightheaded
  • Tired
  • Nauseous
  • Nerve pain
  • Neck pain
  • Headaches
  • Vision problems
  • Lack of appetite
